Our Services
We offer Seven flexible programs designed around you – because everyone deserves care that fits their life perfectly.
1
Self-Directed Personal Care Services – Medicaid
Nevada’s self-directed programs put you in control while receiving care at home.
Your Responsibilities:
• Select your caregiver and approved services
• Hire, train, and manage your caregiver
• Review and approve care hours
What We Handle:
• Payroll, taxes, and all required paperwork (covered by Medicaid)
• Background checks
• Workers’ compensation insurance
The Result: Personalized support with administrative tasks fully managed, allowing you to maintain independence at home.
Eligibility Requirements:
• Currently enrolled in Medicaid including CareSource insurance
• Nevada resident, age 18 or older
• Requires assistance with daily living activities such as bathing, dressing, feeding, toileting, grooming, or transferring
Caregiver Guidelines:
Eligible Caregivers: Adult child, friend, or relative
Not Eligible: Spouse or parent of a consumer under 21

VA Aid & Attendance
Tax-free financial assistance for eligible wartime veterans and surviving spouses who need help with daily living activities.
Paid in addition to the standard VA pension.
Designed to reduce the financial burden of long-term care.
Who It Helps With:
• Bathing

• Meal preparation
• Mobility assistance
• Other daily living activities
How the Benefit Can Be Used:
• In-home care services
• Assisted living communities
• Nursing home care
Eligibility Requirements:
• At least 90 days plus one day of wartime service
• Medical need for assistance with activities of daily living
• Must meet income and net worth guidelines

Long Term Care Insurance
Your Long-Term Care Insurance (LTCI) policy helps cover the cost of quality in-home care while protecting your life savings.
These policies typically cover personal care services not included under traditional health insurance, Medicare, or Medicaid.
Eligibility:
Most LTCI policies require certification from a licensed healthcare professional confirming the need for care due to:
• A cognitive impairment, such as Alzheimer’s disease
• An inability to perform at least two of the six Activities of Daily Living (ADLs)

Nevada Waiver Programs
Nevada’s Medicaid-funded waiver programs allow eligible individuals to receive care at home instead of in a facility.
You remain in control by choosing your caregiver, setting your schedule, and directing your daily support. Assistance is also available to help you find the right caregiver match.
Physical Disability Waiver Qualifications:
• Nevada resident, age 18–64
• Diagnosed with a qualifying physical disability
• Requires a nursing facility level of care
• Chooses to live safely at home
Frail Elderly Waiver Qualifications:
• Nevada resident, age 65 or older
• Clinically frail or at risk of nursing home placement
• Meets financial and medical eligibility requirements
Structured Family Caregiving Qualifications:

Medicaid Eligibility: Must be enrolled in NV Medicaid or an HCBS waiver; meet income/asset limits.

Nursing Home Level of Care: Functional assessment by case manager or DHHS confirms need for nursing facility care.

Functional Limitations: Requires hands-on help or supervision with ≥3 ADLs (e.g., bathing, dressing, eating, mobility).

Qualifying Condition: Program is for dementia or related cognitive disorders.

Living Arrangement: Must live full-time in a private residence with a primary caregiver (family member allowed per program rules).

In-Home Care Assessments & Wellness Checks
In-Home Assessment – A one-time visit by a trained caregiver to observe daily living needs (bathing, dressing, meals, meds, mobility). Not a medical exam.
Goal: Identify safety concerns, understand support needs, and provide documentation for care planning, benefits, or legal purposes.
Wellness Check – A short 30-minute visit for safety and well-being. Includes medication checks, safety scans, status updates, and family communication.
Who comes? A trained, background-checked, CPR-certified caregiver. They observe and report – they do not diagnose or treat.
Goal: Perfect for loved ones who just need a quick check-in to make sure everything is okay—and for out-of-town family who want peace of mind knowing someone stopped by

GUIDE

GUIDE (Guiding an Improved Dementia Experience)
Free Medicare program for dementia care navigation.
Includes 24/7 support, care coordination, caregiver education, home safety visits, and respite care (up to $2,500/year).
Eligible if you have a dementia diagnosis, traditional Medicare (Parts A & B), and are not in a nursing home or hospice. Contact a GUIDE participant to enrol

Coverage: Must be enrolled in traditional Medicare (Parts A and B). (Medicare Advantage enrollees are currently excluded).
