In-Home Functional Assessment
A comprehensive evaluation of daily living needs, home safety, and overall well-being.
What Is an In-Home Assessment?
An in-home assessment is a one-time visit where a Care Assessment Specialist from Elevated Home Care comes to your home to observe and evaluate your daily living needs. This is not a medical exam. It is a functional observation to understand how you manage everyday tasks like bathing, dressing, meal preparation, medication reminders, and mobility.
The goal is to:

Identify any safety concerns in the home

Understand what kind of support may be helpful

Provide documentation for care planning, Medicaid, VA benefits, or legal purposes
What’s included:

Activities of Daily Living (ADLs): bathing, dressing, toileting, eating, transferring, and mobility

Home safety evaluation: fall risks, lighting, clutter, grab bars, and smoke detectors

Cognitive and emotional check: memory, confusion, mood, and social engagement

Caregiver support assessment: family stress and respite needs

Medication review: organization, adherence, and potential risks

30-Minute Wellness Check
A quick, focused visit to ensure safety, medication adherence, and peace of mind.
What Is a Wellness Check?
A wellness check is a short, scheduled visit (typically 30 minutes) to ensure safety and well-being. It is ideal for seniors who live alone or have family out of state.
A wellness check may include:

Safety scan (fall hazards, lighting, clutter)

Quick status check (any falls, pain, confusion, or changes in health)

Reassurance and companionship

Brief update to family members

Who Will Come to My Home?
A trained caregiver from Elevated Home Care will conduct the visit. All caregivers and specialist are:
