We understand that not everyone can visit our office. You can complete your EAP application online from the comfort of your home.

What you will need:

  • All required documents (ID, lease, utility bills, income proof)

  • A clear photo or scan of each document

  • A few minutes to fill out the application

  • You can email all required documents if needed at contact@elevatedhomecare.info

 

What happens next:

  1. You submit the form.

  2. We review your application within 1 business day.

  3. If anything is missing, we will contact you.

  4. We submit your complete application to the State of Nevada.

What to Bring to Your Appointment 

We cannot begin your appointment without ALL of these documents. Please gather everything before you come or email documents to contact@elevatedhomecare.info

1. Photo ID (for the Head of Household)

Bring one of these:

  • Driver's license (any state)

  • State ID card

  • U.S. passport

  • School ID with photo

  • U.S. military card or draft record

  • Military dependent ID card

  • Tribal ID with photo

3. Proof of Where You Live

If you rent:

  • Complete lease agreement (all pages, all names listed, signatures from you AND your landlord)

If your lease does NOT list everyone:

  • A signed letter from your landlord with their phone number AND the names of everyone living in your home

If you own your home:

  • Most recent mortgage statement

5. Income for EVERYONE in Your Home (last 30 days)

If you have a job:

  • Paid weekly: Bring your 4 most recent pay stubs.

  • Paid every two weeks: Bring your 2 most recent pay stubs.

  • Paid monthly: Bring your 1 most recent pay stub.

  • Started a new job: Bring a letter from your employer with your hire date, pay rate, and hours.

If you are self-employed or work temporary/seasonal jobs:

  • Self-employed: Bring 12 months of profit and loss statements.

  • Temporary or seasonal: Bring 12 months of earnings history.

If you receive unearned income:

  • Social Security, SSI, VA benefits, or pension: Bring your current award letter.

  • Unemployment (from a state other than Nevada): Bring your award letter AND proof of your last 2 quarters of earnings.

  • Child support (from a state other than Nevada): Bring a current court order OR a printout showing 12 months of payments.

  • Cash gifts or help from family/friends: Bring a signed letter from each person giving you money. The letter must include their name, address, phone number, the amount they give you, how often they give it, and their signature.

2. Proof of Citizenship (if anyone in your home was                                born outside the U.S.)

Bring one of these for each non-U.S. citizen:

  • U.S. passport

  • Green card (Resident Alien Card – I-551)

  • Naturalization certificate (N-550 or N-570)

  • Citizenship certificate (N-560 or N-561)

4. Your Most Recent Utility Bills

  • Electric bill (NV Energy or other provider)

  • Gas or heating bill (Southwest Gas or other provider)

If you do NOT use NV Energy, Southwest Gas, or Sierra Pacific:

  • 12 months of bills showing usage and dollar amounts for each month

If the utility bill is NOT in your name:

  • A signed letter from the person on the bill authorizing you to apply (include their address and phone number)

6. If Your Bills Are Higher Than Your Income

  • Be prepared to explain how you are paying your living expenses (savings, help from family, etc.)

Important Reminder

Please follow these simple rules to ensure your appointment goes smoothly:

  • ✅ Bring ALL documents listed above.

  • ✅ Bring actual documents or clear photos.

  • ✅ Make sure your lease has ALL household members listed.

  • ✅ Call us if you are missing anything.

 

Avoid these common mistakes:

  • ❌ Showing up without documents – we cannot start your application.

  • ❌ Bringing only handwritten notes.

  • ❌ Bringing a lease with missing names.

  • ❌ Wasting a trip with incomplete documents.

Frequently Asked Questions

How much does this service cost?
Nothing. Our EAP intake service is free. The State pays us to help you.

 

Do I need to qualify for Medicaid to apply?
No. EAP has its own income guidelines (see chart above). You do not need to be on Medicaid.

 

Can I apply if my utility bill is not in my name?
Yes. You will need a signed letter from the person on the bill authorizing you to apply.

 

How long does the appointment take?
About 20 minutes.

 

Can you come to my home if I cannot travel?
We are currently offering in-person appointments at our office and phone appointments for those who cannot come in. Please call us to discuss your options.

 

What if I am missing a document?
We can help you figure out what to do. Call us before your appointment.
